Drawing of an axial section through the head shows a medulloblastoma in a typical location within the midline of the posterior fossa. On imaging studies, medulloblastoma commonly appears heterogeneous, due to areas of hemorrhage, cyst formation, and occasional areas of calcification, as depicted here.

They most commonly present as midline masses in the roof of the 4 th ventricle with associated mass effect and hydrocephalus. Very large mass located centrally within the posterior fossa. It is high T2 signal and low T1 signal with only patchy contrast enhancement. No normal vermis can be identified and the mass is likely to arise from it, completely effacing the fourth ventricle and abutting the dorsal aspect of the brainstem without convincing evidence of extension into the pons.

Medulloblastoma is the most common malignant pediatric brain tumor, accounting for 40% of childhood tumors in the posterior fossa. 1 Genomic characterization of medulloblastoma has recently demonstrated that medulloblastomas can be subdivided into 4 distinct molecular subgroups: wingless (WNT), sonic hedgehog (SHH), group 3, and group 4. 2 ⇓– 4 These subgroups have shown different clinical behaviors and may benefit from subgroup-specific treatments.

Noninvasive imaging techniques, such as X-rays, magnetic resonance imaging and sonography, are valuable tools to help physicians diagnose and treat injuries or illnesses.

Background: Imaging diagnosis of medulloblastoma recurrence relies heavily on identifying new contrast-enhancing lesions on surveillance imaging, with diffusion-weighted imaging (DWI) being used primarily for detection of complications. We propose that DWI is more sensitive in detecting distal and leptomeningeal recurrent medulloblastoma than T1-weighted postgadolinium imaging.
